Adult and Child Services:
- Psychiatric Services
A licensed psychiatrist is available for evaluations and medication management.
- Psychological Services
A licensed psychologist is on staff to provide psychological testing and evaluations.
- Diagnostic Assessment
An intensive clinical and functional face-to-face evaluation of a
recipient s mental health, developmental disability, or substance abuse
condition that results in the issuance of a Diagnostic/Assessment
report with a recommendation regarding services and provides the basis
for the development of a Person Centered Plan (PCP). It also evaluates
the recipient s level of readiness and motivation to engage in
treatment. Results from a Diagnostic/Assessment include an
interpretation of the assessment information, appropriate case
formulation and an order for immediate needs and the development of a
Person Centered Plan.
- Outpatient Therapy
Triumph clinicians can provide assessment, individual, and/or family
therapy. This service can be provided in the home or in an office
setting.
- Community Supports
Consists of mental health and substance abuse rehabilitation services
and supports necessary to assist the client in achieving and
maintaining rehabilitative, sobriety, and recovery goals. These
individuals provide coordination and oversight of initial and ongoing
assessment activities, initial development and ongoing revision of
Person Centered Plans, monitoring of implementation of the plan, as
well as coordination of movement across levels of care, directly to the
person and their family, and coordinates discharge planning and
community re-entry following hospitalization, residential services and
other levels. Community Support Workers provide skill building
including daily and community living skills, socialization skills,
adaptation skills, development of leisure time interests/activities,
symptom management skills, wellness education, psychoeducation and work
readiness skills.
Adult-Specific Services:
- Community Support Team
An intensive community rehabilitation service for adults that provides
treatment and restorative interventions to assist individuals to gain
access to necessary services, reduce psychiatric and addiction symptoms
and develop optimal community living skills.
The service activities include: assistance and support for the individuals
in crisis situations, service coordination, psycho-education and
support for individuals and their families, individual restorative
interventions for the development of interpersonal, community coping
and independent living skills, development of symptom monitoring and
management skills, monitoring medication and self medication.
Youth-Specific Services:
- Intensive In-Home
Addresses the identified needs of children and adolescents who are transitioning
from out of home placements or are at risk of out-of-home placement and
need intensive interventions to remain stable in the community.
The service is delivered primarily to youths in their family's home
with a family focus to diffuse current crisis, evaluate its nature and
intervene to reduce the likelihood of a recurrence, ensure linkage to
needed community services and resources, provide self help and living
skills training for youth, provide parenting skills training to help
the family build skills for coping with youth's disorder, monitor and
manage the presenting psychiatric and addiction symptoms and work with
caregivers in the implementation of home-based behavioral supports.
